Achilles and calf
Achilles and calf physiotherapy, Hobart
Tendons need the right load, not just rest.
Tight like a constant pump, sore first thing in the morning, or cooked after a big hike. We find what's overloading it and build its capacity back up, step by step.

A sore tendon usually needs the right load, not complete rest. Rest can settle the pain, but on its own it doesn't build what the tendon needs to cope.
Tendon, muscle, or both.
Achilles and calf problems usually show up as pain at the back of the heel or in the calf, often worse when you first get up, or after running, hiking or jumping. Some are tendon problems that build up over weeks. Others are a sudden strain. They're loaded differently, so we work out which one you've got first.
Achilles pain
Stiff and sore first thing, eases as it warms up, then worse again after activity.
Calf strain
A sudden pull or pop mid-run or mid-game.
After a big week
Flared up after a long hike, a new running block, or a change in shoes or surface.
Calf and shin
Tight like a constant pump, limiting how far you can run.
What's overloading it.
When the calf muscles can't handle the work, load moves to the tendon. Tendons have less blood supply and recover more slowly, so they're often the first warning sign.
Training spikes
More distance, more hills or more speed, faster than the tendon can adapt.
Calf capacity
Not enough calf strength and endurance for what you're asking of it.
Foot and ankle control
How the foot and ankle handle landing and push-off.
Hip control
Hips that don't control the leg hand more load down the chain.
The other 23.5 hours
Standing, footwear and recovery between sessions.
Previous injury
An old calf or ankle injury that never fully rebuilt.

How we work through it.
We settle it with the right load, then build the calf and tendon back up until it handles the run, hike or game you came in for.
Assess
We test your calf, Achilles, ankle and hip, and how you land and push off.
Explain
Tendon or muscle, what's overloading it, and what that means for loading it.
Plan
A clear pathway to the run, hike or sport you came in to get back to.
Load
Calf and tendon loading, progressed step by step.
Measure
We re-test calf raises, hopping and pain with loading.
Progress
Faster and heavier loading, then back into running, hiking or sport.
Three phases. Your pace.
Calm
Settle the tendon or muscle with the right amount of load. You learn what flares it and what eases it.
Coordination and capacity
Build calf capacity and control, then hopping, landing and faster forces.
Challenge
Strength for life, so your calf and Achilles handle the trails, the track or the field.
What we measure.
A generic example of what we track. Your starting point is your own, which is why we measure rather than guess.
| Measure | Baseline | Re-test | What decides the next step |
|---|---|---|---|
| Single-leg calf raises | Fewer reps, pain near the end | Reps and pain against your other side | Closing the gap moves you into hopping |
| Hop and landing | Not yet, or painful | Control and pain with hopping | Pain-free hopping moves you into running drills |
| Morning stiffness | How long it takes to ease | How long it takes now | Less stiffness means we add load |
Keep building once the pain's gone.
Phase three happens in our own gym. When you're ready, Strength By Design is where that work carries on, coached and physio-designed.
What makes us different.
We look at how you move
Not just where it hurts. The movement pattern, the coordination, and what's actually loading the sore spot.
Strength is the treatment
Pain relief is the start. Loading the right muscles in the right order is the work that follows.
We explain the why
You leave every session knowing more about your body than when you arrived.
Progress you can see
We measure where you start and re-test as you go, so you always know where you're at.
One standard of care
Every physio works to the same assessment-led method, every session.
Clinic and gym under one roof
Seven treatment rooms and a private gym, run by the same team.
Common questions about Achilles or calf pain.
Should I stop running?
Usually not completely, but you may need to change how much and how fast. Tendons respond to the right load. We'll find what your Achilles can handle now and build from there.
My scan shows tendon changes or a partial tear. Do I need surgery?
Not necessarily. Tendon changes are common on scans in people without pain, and many are managed with progressive loading. We'll walk you through what yours means for your situation.
Why is it worse in the morning?
A sore tendon often stiffens overnight and loosens as it warms up. That pattern helps us understand what's going on, and it's one of the things we track as you progress.
I've had calf or Achilles treatment before and it came back. What's different here?
Often the pain settled before the tendon's capacity was rebuilt. We measure your calf strength and hopping as you go, and keep building past the point the pain eases.
Do I need a referral?
No. You can book directly without a GP referral, unless your insurer or GP has specified otherwise. If you have a GP care plan (CDM or EPC), we accept it: you pay the full fee, we lodge the Medicare claim for you, and Medicare pays the rebate to you.
