Why Does a Physiotherapy Clinic Need a Full Gym?
At some point, good rehabilitation should start looking less like treatment and more like your real life.
People ask us this all the time when they first walk into By Design. There are treatment rooms on one side, then a full strength facility on the other: squat racks, machines, weights and cardio equipment. But that combination is very deliberate (‘by design’), because we don’t see physiotherapy and strength training as two completely separate things.
When someone first comes to us in pain, the priority is usually to CALM things down and give them some control back. Pain might be sharp, constant or easily aggravated, and you might not really know what you should or shouldn’t be doing. This first phase is about understanding your triggers, getting symptoms to a more manageable level and helping you feel confident that you know what to do when they appear.
But even here, movement matters. We don’t necessarily need to wait until you’re completely pain-free before we start using your body again. We identify what you can safely continue doing, what might need to be modified or avoided for the time being, and importantly, what the plan is to comfortably get back to those things again.
Once things are more manageable, rehabilitation starts blending into strength training quite quickly. This is where we start rebuilding coordination and capacity, By Design’s Phase 2.
Instead of only thinking about the painful area, we look at how the joints and muscles around it work together and gradually build movement back in. We want the system coordinating as a team. Once we’re happy with how you’re moving, we can start building greater capacity into those movements.
A squat might become a lunge. A lunge might become a step-up. Then we might introduce a hinge, more load, greater range or more complexity. We can manipulate weight, volume, range of motion and difficulty to progressively challenge your body and give it a reason to adapt.
This is where one of our favourite principles comes in:
Movement varies by degree, not kind.
A squat might mean standing up from a chair without using your hands for one person and putting a heavy barbell on their back for another. Both humans need the ability to produce force and control their body through that movement pattern.
What changes is where we start, how much we ask of you and where your finish line needs to be.
And that’s exactly why we have a GYM.
As you become stronger, your rehabilitation should progressively start looking like the thing you’re actually trying to return to.
If you train, your rehab should start looking like training. If you play sport, it should begin reflecting your sport. If you don’t train at all, it should start looking like your real life: carrying things, climbing stairs, getting off the ground, gardening, working or keeping up with your kids.
Eventually, the goal is our third phase: Challenge or Compete.
That doesn’t mean everyone needs to become an athlete. It means you’ve built enough capacity to CHALLENGE your body and confidently do the things you actually want to do. Ideally, you’ve also developed a sustainable way of continuing to train or move so that the progress you’ve made doesn’t disappear the moment physiotherapy finishes.
That’s why our physiotherapists spend so much time on the gym floor. They’re coaching movement, introducing load, watching how people respond and progressing exercises as someone’s capacity improves. We don’t want to get you feeling better and then send you somewhere else to figure out the rest.
The treatment room and gym floor are simply different parts of the same process. And throughout your rehabilitation, we’ll move between them constantly depending on what you need.
Because rehab shouldn’t only prepare you for your next physio appointment.
It should prepare you for your LIFE.
